Policy and Procedure

At the arrival

Intake form will be reviewed and updated before each session.

Informed Consent

Prior to your first massage session, treatment plan will be discussed with you. You will be asked to sign the consent that you have read and understood the policy and procedure and agreed to comply with them. The consent will be renewed annually.

Scope of Practice

I offer a professional therapeutic massage. I do not diagnose medical conditions or prescribe any type of medication/exercise. I may refer you to medical professional for specific concerns, but it does not substitute medical attention.

Confidentiality

The client’s information, condition, and the visit will be confidential. Your records will be released to third parties only with your written consent. Record may be surrendered by court orders.

Health History

The client is responsible for informing the massage therapist about the existing or new medical conditions at each massage session. Medical history form will be updated and signed annually. If contraindication to the planned massage exist, alternative plan will be discussed, or massage will be postponed until cleared by a medical doctor and written statement is to be provided before next massage session. In the event of postponing the session due to medical conditions, the session will not be charged. The client will inform the therapist of any lesions or breaks in the skin before the session; the conditions will be discussed and those areas will be avoided as needed.

Hygiene

All clients are expected to exhibit good hygiene before the massage session. Please minimize the use of perfume and heavily scented personal products.

Respect for clients’ needs and boundaries

My clients’ comfort is the most important and their request will be fulfilled within my scope of practice and policies.

The massage therapist will leave the room while you undress. The client can remove the clothing as needed to their comfort level. The client will be draped during massage session and only the area being massaged will be undraped.  Clients have a right to request to modify massage technique, pressure, environment such as room temperature and music anytime during the session. You may refuse or ask to stop any massage method at any time.

Professional boundaries

Inappropriate or disruptive behavior will not be tolerated. Sexual innuendos or discussion of any kind between the client and the massage therapist is never appropriate. When that occurs, the session will be terminated immediately, and the client will be responsible for full payment.

Massage session for minors

The age of 18 and under can receive a massage with written parental permission.  An accompanying adult is asked to remain in the massage room or in the waiting area and are allowed to come in the massage room anytime during the session.

Clients under the influence

Client may not be under the influence of drugs or alcohol at the time of the massage session. In this case massage will be postponed, and full price will be charged.

Smoking and chewing

Smoking including e-cigarettes and chewing tobacco or gum are not permitted on the premises.

Lost items

The massage therapist is not responsible for any lost personal items in the massage room. Please utilize the area and trays provided to place your personal belongings including fragile jewelry and watches.
